PROPOSAL – Primary Health Network (PHN) Measuring for Improvement Program
This Proposal as defined in and for the purposes of the Head Agreement – Program Services between the Improvement Foundation (Australia) Limited ACN 122 939 299 (IF) and the Health Service as specified above and which has a Head Agreement in place with IF.
Terms defined in the Conditions have the same meaning when used in this Proposal.
Primary Health Networks (PHNs) have been established with the key objectives of:
- Increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes; and
- Improving coordination of care to ensure patients receive the right care in the right place at the right time.
To achieve these objectives PHNs will implement a range of activities and the Health Service may be invited to participate in such. Where the Health Service agrees to participate in an activity this may be subject to an agreement between the PHN and the Health Service.
Measuring for Improvement Program (MI Program)
The Health Service has agreed to participate in the North Coast PHN (Health Body) MI Program. This proposal documents the terms on which IF will provide the Health Service with access to IF’s web portal for use by the Health Services as part of its participation in the Program.
Under the MI Program the Health Body will have access to data submitted by the Health Service to IF’s web portal. At a high level, these data will be available to the Health Body for planning improvement activities, monitoring improvement over time and reporting outcomes from the region. Access and use of data are further defined in Clause 4.
Role of Parties – under this proposal IF will provide the Services.
1. Term (when the Services are to start and be completed).
Commencement Date: IF and the Health Service will commence providing the Services on the date this agreement has been signed.
Completion Date: No completion date is specified and the term of this Proposal is subject to Term and Termination Clause in the Head Agreement.
As part of the PHN MI Program, the Health Body will encourage Health Services to submit data over time for the purposes of PHN planning, monitoring and reporting improvement in the delivery of primary health care services.
Health Services will, amongst other things, submit data to qiConnect and may work collaboratively with the Health Body to identify opportunities for improvement and monitor improvement over time. The Health Body is responsible for the conduct of the MI Program and will provide support to the Health Services from time to time as required by the MI Program. IF is responsible for providing specialist advice and expertise to the Health Body to conduct the MI Program.
IF will develop and deliver a purpose specific web portal site that will be hosted within qiConnect. Health Services may submit data to qiConnect and have access to the web portal site to view a range of reports and graphs. The Health Service will also have access to a range of quality improvement functions and features in qiConnect and in the Canning Data Extraction Tool (the Canning Tool).
The Health Service will be enrolled in the qiCommunity for access to quality improvement functions, features and resources within qiConnect. The Health Service can also elect to receive automated reports (via email) from qiConnect that show the PHN’s progress to achieving outcomes in national and regional health priorities.
Where the Health Service is involved in other programs within qiConnect, these programs will be accessible through the Health Service home page.
4. Data Access and Acknowledgement
The Health Body and its employees will have access to the Data collected under this Proposal for the purpose of implementing the MI Program, along with IF staff for Program implementation support and advice to the Health Body.
The Authorised User of the Health Service is authorised to make changes to information for the Health Service account in relation to this MI Proposal including the addition and/or removal of other person/s.
Service Administrators will have access to the Data collected under this Proposal along with IF staff, for administration purposes.
4.2 Data Use
For the purpose of this MI Proposal Data means all data provided to IF by a Health Service.
For the purposes of this MI Proposal, Data is submitted by the Health Service on the understanding that Data may be used for the following purposes:
- The Health Body will have access to individual Health Service Data for the purpose of planning, monitoring and reporting improvement in the delivery of primary health care services.
- Neither IF or the Health Body will own submitted Health Service Data. The Health Service submitting such data retains ownership.
IF has rights to access the Data for the provision of technical development, administration and support to the MI Program in this Proposal.
The Health Body will aggregate Data from participating Health Services to ensure that Health Services are not identified. Where participating Health Service Data is aggregated, the Data will not identify the Health Service as the source of the same unless the Health Service expressly agrees otherwise.
IF may use the aggregated de-identified Health Service level data for the purposes of carrying out the Agreement, benchmarking in the qiCommunity and promoting quality improvement. Where participating Health Service Data is aggregated, the Data will not identify the Health Service as the source of the same unless the Health Service expressly agrees otherwise.
The Health Body may use aggregated de-identified Health Service level data with regional and national health agencies for the purpose of improving health outcomes and service delivery. Data will be aggregated at an appropriate level to ensure the identity of the Health Service cannot be identified as the source of the same unless the Health Service expressly agrees otherwise.
