The Improvement Foundation (IF) is to deliver a range of new Australian Primary Care Collaborative (APCC) Program Waves as part of the current phase of the Program.

Practice & Health Service Manager Lead Program
IF is delivering a specialised Program focussed on greater engagement of practice managers to lead quality improvement work within their organisations.

This is a valuable training opportunity for practice or health service managers and can deliver significant benefits to the whole practice through increased efficiencies, maximised income streams and improved systematic patient care.

Participation is FREE, and IF will provide direct support to participating general practices and health services.

IF are working closely with the Australian Association of Practice Managers to ensure the Program’s design and delivery adequately supports participants to implement quality improvement initiatives and improve patient outcomes within their organisations.

Practice Nurse Lead Program
IF is delivering a specialised Program focussed on greater engagement of practice nurses to lead quality improvement work within their organisations.

This is a valuable training opportunity for practice nurses and can deliver significant benefits to the whole practice through increased efficiencies, maximised income streams and improved systematic patient care.

Participation is FREE, and IF will provide direct support to participating general practices and health services.

IF is working closely with the Australian Practice Nurses Association to ensure the Program’s design and delivery adequately supports participants to implement quality improvement initiatives and improve patient outcomes within their organisations.

Integrated Care Wave
The Integrated Care Wave will be delivered in collaboration with the local Health and Hospital Service in Queensland.

This Program will engage Primary Care Health Services to improve the management of patients with complex chronic disease, and support improved integration between the primary healthcare system and Hospital and Health Service network.

It is expected that a secondary outcome of this work will be a measurable reduction in avoidable hospital admissions and an increase in the use of the national eHealth infrastructure.
Participation is FREE, and direct support will be provided to participating general practices and health services.
