The value of accurate information on safety and quality in health care for clinicians, policy makers and PHNs is certain, however, the availability of standardised measurement sets for reporting of major diseases across primary and secondary health care sectors remains fragmented.

With over ten years expertise in the sector, we haven risen to this challenge by establishing the new Australian Primary Care Collaboration (APCC). This Collaboration consists of membership from leading health care organisations to ensure the consistent collection of national quality and safety health care measurement sets, supporting front line teams to improve the delivery of effective, safe and efficient patient-centred care.

Working Collaboratively with PHNs

Our current measurement suite is extensive and, through this Collaboration, can be continually enhanced with additional data sets that are informed by requirements of peak health care bodies, including PHNs. Work has already begun with Northern Sydney PHN to develop a comprehensive set of quality improvement disease indicators for dementia. Through the APCC, an Expert Reference Panel will be convened to provide subject matter expertise for the development of a national set of dementia indicators. With our proven experience in developing national disease indicators, the new dementia indicators will be a valuable resource for PHNs and health services across Australia.

Additionally, Medical Director’s upcoming product release of ‘MD Insights’ will ensure that all of our quality and safety measurement sets will be automatically and directly submitted into qiConnect, our online and secure quality improvement portal.  This new update will make it easier for users to submit data without having to purchase or rely on traditional data extraction tools.

Get Involved

PHNs are invited to get involved in expanding the national measurement set. By working together, we will be able to identify measurement gaps and develop suitable indicators so that data can be shared in a nationally consistent format amongst primary and secondary care. Your support of the APCC will provide the health care sector with a consistent framework to measure the improvement of chronic diseases, improving the safety and quality of health care across the nation.
