Closing the Gap: Measure and Act is an Improvement Foundation (IF) initiative. We are inviting General Practices around Australia to collect, submit and measure data on key indicators that could lead to better health outcomes for Aboriginal and Torres Strait Islander peoples.
It all starts by asking your patients one crucial first question, ‘Are you of Aboriginal or Torres Strait Islander origin?’ Then record the response in the practice’s clinical software.
