Integrated Care Collaborative
To help achieve a more integrated health system with a focus on patient centred care, the North Coast PHN and Northern NSW Local Health District are implementing an Integrated Care Collaborative supported by the Improvement Foundation. Leveraging a decade of quality improvement experience from the Australian Primary Care Collaborative (APCC) Program, we’re supporting this Collaborative with:
- Specialist quality improvement advice
- Development of localised materials
- Access to existing Collaborative methodology infrastructure
- Ongoing support and training throughout the duration of the program.
The Collaborative will support participating health services and providers to learn from each other while gaining and utilising knowledge from leading experts and practitioners. Participants will have the opportunity to develop, trial and assess the impact of changes being made to ensure that they are improving the patient journey.
Musculoskeletal Primary Health Care Initiative
The Improvement Foundation is working in partnership with the North Coast PHN and Mid North Coast Local Health District to implement the Musculoskeletal Collaborative, which has a focus on acute low back pain and osteoporosis (with particular reference to the re-prevention of fractures). The Collaborative is part of a larger initiative, the Musculoskeletal Primary Health Care Initiative (MSK PHCI), funded by the NSW Agency for Clinical Innovation (ACI).
The objective of the MSK PHCI is to trial the implementation of a sustainable Collaborative and integrated multidisciplinary team approach to the management of specified musculoskeletal conditions. The overall aim is to reduce pain, increase functional capacity and improve the quality of life for people with musculoskeletal conditions who are participating in this initiative.
