Quality Improvement Award Terms and Conditions


The Improvement Foundation (Australia) Limited (ACN 122 939 299) (IF) is pleased to offer two employees from Primary Health Care Organisations the opportunity to learn from quality improvement (QI) experts from around the world at the Improving Healthcare International Convention (Convention), sponsored by the Improvement from Monday 16 to Wednesday18 November 2015 at the Pullman & Mercure Melbourne Albert Park.

These are the terms and conditions for IF’s QI Award (Competition).


1. Who can apply

1.1    Entry is open to all Australian persons over the age of 18, employed at a Primary Health Care Organisation (including General Practice, Aboriginal and Torres Strait Islander Health Organisations, Allied Health Organisations and Primary Health Networks at the time of entry, who has been involved in an IF program / activity and implemented successful QI activities within their work place.

1.2   The Award is open to clinical and non-clinical staff, who meet criteria 1.1.


2. How to enter

2.1   The Competition commences on 29 June 2015 at 09.00 (AEST) and closes on 8 October 2015 at 17:00 (AEST).

2.2   In order to enter the Competition, you must complete the application form (Application) available at www.improve.org.au/qiawards and register your name, address , daytime telephone number, email address, employer, position, select an award category and answer the ‘Competition Questions’. (Application):

2.3   Your answer to the Competition Questions must be your original creation and must be free from any claims, including but not limited to copyright or trademark claims. Where necessary, all information must be properly and fully acknowledged.

2.4   You may submit as many Applications as you like, however, the answer to the Competition Questions in each Application must be unique and original.


3. Prize

 3.1   There will be one (1) winner selected for each award category in accordance with paragraph four (4) of these Terms and Conditions (Successful Applicant).

 3.2   The prize for each winner consists of (Prize):

3.2.1.   One return economy airfare to Melbourne. Departing on Monday 16 November 2015 and leaving Melbourne on Wednesday 18 November 2015. (dates are negotiable upon written request and IF approval) and includes all associated airfare taxes;

3.2.2.   Two (2) nights (only), in a standard hotel room accommodation in Melbourne (Hotel) including breakfast on Monday 16 November and Tuesday 17 November 2015;

3.2.3.   Return transfers from Melbourne Airport to the Hotel;

3.2.4.   One (1) full registration for both days of the Convention, including the official welcome reception; and

3.2.5.   Ticket to attend the Gala Dinner.

 3.3   The total value of each Prize is approximately $AUD 4,500.00 (including GST) but may vary depending on point of departure. IF accepts no responsibility for any change in value of the Prize.

3.4   The Successful Applicant is responsible for all expenses other than those specified in these Terms and Conditions.

3.5   The return flights may include connecting flights to and from one of Australia’s capacity cities.

3.6   Flights and accommodation are subject to availability at the time of booking, are non-transferable and cannot be exchanged for other destinations. All other elements of the Prize are subject to availability and cannot be amended once booked.

 3.7   The Successful Applicant must be able to travel within the travel period specified in paragraph 2.1 of these Terms and Conditions.

3.8   The itinerary for the Prize and all associated information and details will be emailed to the Successful Applicant by 30 October 2015.

3.9   The Application will be considered to have been entered once it is received by IF’s online database. Upon submitting your Application you will be presented with a confirmation notice. To be eligible to win the Prize, your Application must be received before 8 October 2015 at 17:00 (AEST).


4. How to win the Prize

 4.1   All Applications completed and submitted in accordance with these Terms and Conditions will be judged at IF’s office at Level 5, 19 Grenfell St, Adelaide between 15 October to 22 October 2015.

4.2   One (1) Successful Applicant from each award category will be selected by a panel of three (3) judges appointed by IF (Judges). Answers to the Competition Question will be judged with regards to planning, implementing, reviewing and improving change using a QI approach.

4.3   The Judge’s determination of the Successful Applicant(s) will be final and no correspondence will be entered into.

4.4   You acknowledge that this is not a game of skill or chance and that the selection of the Successful Applicants is based solely on merits of the application provided.

4.5   The Successful Applicants will be notified by telephone and email by close of business Tuesday, 27 October 2015. The winners will be published in a special edition of the qiCommunity Update, IF’s newsletter and on the IF website.

4.6    IF reserves the right to request that the Successful Applicant(s) provide suitable proof of age, identity and/or proof of employment with an Primary Health Care Organisation at the time the Successful Applicant(s) submitted his or her Application, prior to awarding the Prize. In the event that the Successful Applicant(s) cannot provide suitable proof, the Successful Applicant(s) will forfeit the Prize.

4.7   If IF is unable to contact the Successful Applicant(s) by 27 October 2015, the Successful Applicant(s) will forfeit the Prize in its entirety and the Prize may be awarded to another applicant at the discretion of the Judges. IF will not be liable for a Successful Applicant(s) who does not respond to IF’s email or telephone contact attempts.

4.8   IF reserves the right to rejudge the Applications in the event that an applicant, who was initially deemed to be the Successful Applicant is in breach of these Terms and Conditions.


5. No liability

5.1   IF including its officers, employees, agents and its associated companies are not liable to the Successful Applicants including but not limited to liability for negligence for any loss, expense, damage, death or injury which is suffered or sustained in connection with all or any of the Prize or accepting or using the Prize, except for any liability which cannot be excluded by law.

5.2   The Successful Applicants should be aware that tax implications may arise as a result of claiming the Prize and should seek independent financial advice. IF is not responsible for any tax implications that may arise from the Successful Applicants being awarded the Prize.


 6. General

6.1   It is your responsibility to notify IF if your contact details change once you have submitted your Application. IF can be contacted at ifaward@improve.org.au or phone 08 8422 7400 or 1800 771 552 (toll free within Australia).

6.2   In the case of the intervention of any outside act, agent or event which prevents or significantly hinders IF’s ability to proceed with running the Competition (including the postponement or cancellation of the Convention) in the manner described in these Terms and Conditions, including but not limited to vandalism, power failures, tempests, natural disasters, acts of God, civil unrest, strike, war, act of terrorism, IF may in its absolute discretion cancel, modify or suspend the Competition.

6.3   IF reserves the right to reasonably amend and vary these Terms and Conditions at its sole discretion, orally and in writing.

6.4   Any intellectual property subsisting in the Applications become and remain the property of IF and will not be returned to you and IF may use the same for any purposes.

6.5   In being awarded the Prize, the Successful Applicant(s) agree to participate and co-operate as required in all editorial activities relating to the Prize, including but not limited to being interviewed and photographed. IF may use the relevant interviews, name, footage and photographs of the Successful Applicant(s) as IF decides and the Successful Applicant(s) will not be entitled to any fee for such use.

6.6   IF may ask the Successful Applicant(s) to participate in webinars and face to face events to share their quality improvement story with others. The Successful Applicant(s) agrees to, wherever possible support and participate in these events. The Successful Applicant(s) will be reimbursed for any reasonable expenses as a result of attending these events, as negotiated with IF prior to the event.

6.7   The Successful Applicant(s) agrees that they will not sell or otherwise provide their story and/or photographs concerning their Application or Prize experience to any media or other organisation unless IF provides consent.

6.8   The Prize is non-transferable, non-refundable and cannot be redeemed for cash.

6.9   IF collects your personal information (as that term is defined in the Privacy Act 1988 (Cth)) for the purpose of conducting and promoting this Competition (including, but not limited to, determining and notifying the Successful Applicants). You are advised that, from time to time, IF may disclose your personal information to third parties for the purpose of administering the Competition or for other purposes contemplated in these Terms and Conditions.

6.10   All Applications become the property of IF and your name may be included on a database. By applying for the Prize you consent to IF including your name in future mail, email, SMS or other campaigns. If you do not wish to receive any future correspondence from IF please send an email to IF at ifaward@improve.org.au.
