About our Agreements


agreement1The Head Agreement sets out general terms and conditions, allowing for Proposals (which are relatively brief documents) to be formed underneath it. This structure allows for new Proposals to be easily entered into, should you wish to participate in additional quality programs or initiatives with the Improvement Foundation in the future. This online Head Agreement now provides access to the qiConnect, providing your health service access to continuous quality improvement resources and access to submit and view your aggregated health service data for quality improvement purposes. agreement2All data submitted by your general practice is aggregated at the general practice level and cannot be linked to any individual patient to protect patient privacy. In addition, data displayed on improvement graphs are de-identified and provide no general practice information.


agreement3Our online Agreement has been simplified to make it easier for Health Services to access qiConnect quickly and easily. Health Services who want to submit and view data in qiConnect can now accept the qiCommunity proposal online using this simplified form.


agreement4When the online registration form is submitted, it is reviewed internally for completeness. Once all the details have been entered into our secure database, you will then receive notification of your registration details. Depending on how many additional users have been registered on the form, each user will be sent via email their unique qiConnect username and password.
