Closing the Gap: Measure and Act

Helping Hands artwork

Closing the Gap: Measure and Act is an Improvement Foundation (IF) initiative. We are inviting General Practices around Australia to collect, submit and measure data on key indicators that could lead to better health outcomes for Aboriginal and Torres Strait Islander peoples.

It all starts by asking your patients one crucial first question, ‘Are you of Aboriginal or Torres Strait Islander origin?’ Then record the response in the practice’s clinical software.

Hear Emma Pullen, Practice Manager at Walkerston and Marian General Practice Superclinics, discuss how improving their patient data leads to improved health outcomes.

It’s easy to participate
If you are one of the many practices that are already submitting data to the IF’s web portal qiConnect , there is nothing extra to do.

Don’t have access to qiConnect?
If you do not have access to qiConnect, simply fill in the online registration form to get started. Register online here.
